If you're taking a train/bus/car through Lithuania into Belarus/Latvia - yes, you'll need it. You'll need to apply with Lithuania:
The Embassy is competent to examine and decide on application for Schengen visa for the purpose of transit:
(a) when transit is through the territory of Lithuania only; or
(b) in the case of transit is through several Member States, when an applicant intends to cross Lithuanian external border to start the transit.
If you are taking a direct flight from Kaliningrad to Moscow then you don't.
If you want to avoid the Schengen visa hassle, there's one more option besides flying. The Kaliningrad Oblast is connected with the rest of Russia also by direct ferries from the port of Baltiysk, either to Saint Petersburg or to Ust-Luga (west of Saint Petersburg). English-language information is hard to find, but this site tells that there are 3 crossings a week and the voyage takes 27 hours.
