This is the front of the form (obtained through a FOI request to the Home Office).

The leave conditions are stated on the rear side of the form, with the stamp being placed at the bottom right corner of the rear side.
The UK IS116 document is a form completed by the Home Office for individuals arriving in the UK through schemes like the Ukraine Homes for Ukraine scheme, or the Afghan Citizens Resettlement Scheme (ACRS). It essentially serves as proof of the holder’s immigration status. The IS116 document, along with a passport (if one is available) or the visa stamp, proves that the holder is eligible to work and access various services in the UK, such as healthcare and education
IS116 contains the holder's personal details, including their immigration status and the duration of their Leave to Remain, which is the period they are allowed to stay in the UK. On arrival from eg Ukraine, non-UK nationals will have either a passport which is stamped or an IS116 document provided to them by Visa immigration. If the citizen does not have a current passport, they will be issued with an IS116 document. This includes all children who will have individual IS116 documents.
Current Immigration rules on entering the UK with a national identity card are available here. AFAIK an IS116 would not now be issued at the border other than in the circumstances described above.
